This Assurant Health Dental benefit is a fee for service dental plan that pays cash benefits that help offset the cost of routine, basic and major dental services. With Assurant Health Dental, members can:

• Choose between two different plans, Basic & Plus
• Visit any dentist
• Receive quick cash benefits that are sent directly to you or the provider if preferred.
• Members can keep coverage even if one chooses to discontinue individual or family health insurance coverage.

Coverage includes Assurant wellness services, basic services, major services, Temporomandibular Joint services (TMJ), and a combined annual Assurant Health Dental benefit maximum.

Assurant Dental and Vision Discount Plan is an additional benefit that provides discounts on services from a nationwide network of dental and eyewear providers. Assurant Health members will save 15% to 50% on dental services and 10%-60% on eyewear.
